Health Management Need Indicator (HMNI)
Documented clinical best practice guidelines exist for many health management scenarios. These guidelines are comprised of specific steps that, if followed, will facilitate the achievement of optimal health. We define each specific step as a Health Management Need Indicator (HMNI). We have then broadened the scope and breadth of HMNIs, to include assessments, social determinants and care coordination activities. This has enabled us to elevate the HMNI into a defined term component within a complex healthcare system structure. Each HMNI is delivered through a patient Care Plan Ledger (CPL) clinical language that describes specific health management work effort needs.
